Evidence-Based Exercise for Veterans’ Health, Recovery, and Functional Independence

At Achieve Exercise Physiologists, we proudly support DVA clients by delivering personalised, clinically-supervised exercise physiology services tailored to the unique needs of veterans and entitled persons. EPC Exercise Physiology Services

Medicare-Funded Exercise for Chronic Disease Management and Functional Health

At Achieve Exercise Physiologists, we deliver expert, personalised care under the EPC program (now formally known as the Chronic Disease Management [CDM] Plan) for eligible clients referred by their GP. These Medicare-subsidised exercise physiology sessions are designed to help individuals better manage chronic conditions, improve functional independence, and enhance overall wellbeing.

All services are provided by ESSA-accredited Exercise Physiologists, with tailored treatment plans based on your individual needs and your GP’s referral.


What We Offer
